Linking vulnerable residents to resources and services

Removing barriers to care is one of the foundational ways Providence Holy Cross Medical Center supports the San Fernando Valley. Through the Los Angeles County Community Public Health Teams (CHPT) project, Providence Community Health deploys trusted community health workers (CHWs) to neighborhoods and communities to connect residents to essential services.

This new model of public health service delivery meets community members where they are  providing vital access to resources. By utilizing a wide network of like-minded organizations, the CPHT project has created long-term partnerships with community-based organizations and health care partners, expanding the public health workforce and building infrastructure for a community-based system of care.

Each household that community health workers visit has unique and individual needs. After completing a household assessment with a community member, the CHW and community member work together to identify what services are needed and how CPHT can provide support.

"The impact of CPHT is rooted in community: listening to what families identify as their needs, honoring their lived experiences and working alongside them to build healthier, more resilient neighborhoods,” program manager Stephanie Bermudez said.

Alongside our partner, San Fernando Community Health Center, the CPHT project served over 6,000 households and completed over 300 referrals to essential services in 2025.
