Easing the way: Findhelp connects thousands to the resources they need

In 2025, Providence advanced its efforts to address the social drivers of health by launching Findhelp, a community resource platform designed to make it easier for patients and caregivers to find and connect with essential support services. From housing and food assistance to transportation and health-related resources, Findhelp helps transform how social needs identified in care settings translate into real-world support.

Following its systemwide launch, Findhelp saw rapid and sustained growth in patient engagement—particularly after the introduction of a MyChart entry point that enabled patients to search for resources directly and anonymously. More than 60,000 patients used Findhelp in 2025 to explore available community supports and make meaningful connections. This surge in patient engagement reinforced what caregivers see every day: the need for accessible, reliable pathways to social services is both significant and ongoing. 

Caregivers across Providence use Findhelp to ease the way for patients navigating complex social challenges. Care/case managers, social workers and community health workers use the tool to quickly identify local resources, share information with patients and document follow-up actions—supporting high-quality, whole person care. Collectively, caregiver use of Findhelp in 2025 resulted in thousands of searches and connections to local organizations. 

Findhelp’s impact was felt across regions and settings. While patient needs vary across geographies, common themes emerged: housing, food access, transportation and health-related services consistently ranked among the most frequently searched needs. These trends underscore the foundational role that social supports play in health outcomes and the importance of making those supports easier to find and access. 

Beyond individual interactions, Findhelp also lays the groundwork for stronger community partnerships. By tracking searches and connections at scale, the platform generates insights that can inform how Providence can partner with community based organizations, identify gaps in local resource availability and strengthen networks of care. In this way, Findhelp not only supports individual patients in the moment, but also contributes to broader efforts to build community resilience and advance health equity. 

As Providence looks ahead, Findhelp’s 2025 success represents both impact and momentum. The platform demonstrates how technology, when thoughtfully integrated into care, can reduce barriers, support caregivers and empower patients to take an active role in meeting their social needs.  

With plans to expand access, deepen community engagement and continue innovating in 2026, Findhelp is ensuring that when social needs are identified, support is not far behind.
