Advance care planning helps ensure your medical wishes are known and honored if you are unable to speak for yourself. Making decisions now can ease stress and uncertainty for loved ones later. As caregivers, we have the opportunity to model for our community by taking steps to get our own documents prepared and encourage others to do the same.
The St. Peter Patient Experience & Health Equity Council, in partnership with Palliative Care, will host an educational table April 16 to support caregivers in understanding and completing advance care planning documents. Caregivers can learn about advance directives, select a health care agent and fill out required forms.
Palliative Care Medical Director Dr. Juliette Erickson said advance care planning “is a gift for your loved ones, allowing for clarity and ensuring your values guide decisions when your voice cannot.”

Hours: 5–8 a.m. and 11 a.m.–2 p.m.
Educational materials and help with Durable Power of Attorney for Health Care forms will be available. Caregivers completing a form should bring identification.
