SOUTH PUGET SOUND — Flu season is approaching, and flu vaccination clinics will begin across our campuses Sept. 14. Medical staff providers are encouraged to get vaccinated early to help protect patients, fellow caregivers, visitors and our communities while supporting our shared commitment to patient safety and regulatory compliance.
All clinicians with privileges must obtain a seasonal influenza vaccination annually or submit a completed declination form to the Medical Staff Office. Documentaiton is required by Oct. 31.
How to submit your flu documentation

- Completed Influenza Attestation Form (below)
- Completed Influenze Declination Form (below)

Return your flu vaccine record, completed attestation or declination form by email to Kim Packer or fax to (360) 493-5226. Questions may be directed to the Medical Staff Office at 360-493-7360.

- Flu vaccines are available by walk-in and appointment, Monday-Friday, 7:30 a.m. -11:15 a.m. and 12:15 p.m. - 3:45 p.m., in the Caregiver Health Office in Cedar House (419 Lilly Road NE).
