Foundation funds help with patient discharge

Short-term support available through Care Management.

More than 20 years ago Providence Southwest Washington Foundation established funds to help remove barriers to patient discharges from our two hospitals. These funds provide support to our patients to allow them to continue the healing process and avoid additional hospitalization.

"Often referenced as the Sr. Rita or Sr. Carolyn funds, Foundation board members and donors have a steadfast commitment to providing help where it can ease the way for both the providers and the recipients," said Chief Philanthropy Officer Peter Brennan.

Over the years, our clinical teams have developed creative, safe, and smart solutions to these discharge barriers ensuring better outcomes, happier patients, and the opening of a bed for the next patient in need of their care. Care Management, under the leadership of Lesley Kinzner, manages these funds.
