Hospital at Home has served more than 200 St. Peter Hospital patients

Thank you to the Providence St. Peter community for continued referrals to the Providence St. Peter Hospital at Home program.

We provide hospital-level care to patients in their own homes.  This helps ease hospital overcrowding and ED boarding.  Patients love getting care in their own home with a 100% patient experience score to date this year.

We love taking patients with CHF exacerbations, COPD exacerbations, PNA, UTIs and cellulitis for hospital-level care in their home.  Please see our “Patients We Love” flyer for details on our “Top 5 conditions.”

We’re looking to increase enrollment to one patient each day and we drop a note in the chart when we’re actively looking at a patient.

We are happy to chat through specific patients at any time.  

  1. You can also secure chat the Command Center nurses at any point (Epic secure chat group “WSP Hospital at Home Command Center Nursing”—available in the “groups” tab in secure chat) and they can ask the doc on to give you a call. 
  2. You can also call the HaH hotline at 833-421-2866 to be connected to the command center nurses or HaH physicians.
