COVID-19 Vaccine Clinic for Long-term Care Workers at Providence Mount St. Vincent

Who: 

Caregivers (Long-term Care Healthcare Workers) are eligible if they have not yet had a dose of 23-24 Formula COVID-19 Vaccine (haven't had a vaccine since before September 2023).

When:

March 7th & March 14th  (both on Thursdays)

12:30pm-3:30pm

Where:

Providence Mount St. Vincent

4831 35th Ave SW

Seattle, WA 98126

NW Room - conference room just inside the West Entrance of the main building, to the left as you enter.

How:

Public Health - Seattle & King County Mobile Clinic Staff will be administering vaccines.

Pre-Registration Required:

Step 1: Register with the Mobile Vaccine Team: http://tinyurl.com/mobilevaxregistration, choose the clinic date Thursday, March 7th, 2024, OR Thursday, March 14th, 2024  - Choose the 2 PM Clinic Time

    • It will indicate the clinic is in Kent, but the Mobile Team will be at PMSV on those dates & times. Also, you can come any time during the PMSV clinic window of 12:30pm-3:30pm on your registered day.
    • Register as a guest. 
    • Insured: use your insurance card info
    • Uninsured: choose this option only if you don’t have health insurance- supplies are limited for this option

Step 2: Sign up and let PMSV know which clinic you will be attending: https://forms.office.com/r/k9PFPJuJay

Step 3:  PRINT and COMPLETE the "Kent Covax NOPP & Consent to Treat and Bill" (single pdf document). The NOPP Policy document is for your review and does not need to be printed. (Link Pending)

Bring your completed Step 3 documents, a photo ID, and your insurance card/info (just in case) when you come to get vaccinated on your registered day.
