How to receive a vaccine

For those eligible, reserve a COVID-19 vaccination appointment or walk-in today

 

Providence St. Mary Medical Center is hosting a San Bernardino County drive-thru vaccination site at Providence St. Mary’s Healthy Communities building, 16000 Kasota Road, Apple Valley.

Anyone interested in receiving the vaccine can schedule an appointment at myturn.ca.gov. Walk-ins are also welcome. All persons age 12 and over are eligible to receive the vaccine.

What to know

As soon as you or your loved ones are scheduled to receive a vaccine, here are some important details to review prior to the appointment:

Vaccine Clinic Location

Providence St. Mary’s Healthy Communities building, 16000 Kasota Road, Apple Valley.

Directions from Interstate 15

Head east on D Street until it turns into Highway 18. Continue traveling east on Highway 18 and turn left on Kasota Road. Make a left into the Healthy Communities building's parking lot on Kasota Road. Signage will direct you to the vaccine site.

*** Please be prepared to present photo ID and proof of San Bernardino County residency to the parking attendant. ***

Things to Bring

Individuals being vaccinated MUST provide proof of vaccine eligibility under the current phases and tiers: 

  • Proof of San Bernardino County residency (physical or digital) 
  • A photo ID

Fast Facts

  • If you must cancel, please do so as soon as possible to help us ensure that no vaccine goes to waste. We must be careful stewards of the vaccine. Due to how the vaccines must be prepared and stored, if you are a no-show, it could result in a vaccine being wasted.
