AdvantAge: The Health Care You Need in the Medicare Years

May is Older Americans Month – and there’s no better time for patients age 62 and older to enjoy customized, high-quality care offered through Providence’s AdvantAge Health Centers, where staff and physicians are dedicated to caring for senior populations for better overall health and a positive care experience. 

These end-to-end health clinics serve any senior over the age of 62, with benefits that include personalized care coordination from physicians and caregivers in a range of areas, including behavioral health support, pharmacy consultations and insurance experts. 

As part of Providence’s continued mission to provide the finest health and wellness care, the goal of our AdvantAge program is to help senior patients age gracefully through preventative and age-appropriate care, with physicians and staff who truly understand the health needs of older adults. 

At our AdvantAge Health Centers, senior patients can receive a full range of primary care services, such as: 

  • Screenings, check-ups and reminders. 

  • Preventive and wellness care. 

  • Managing chronic conditions. 

  • Diagnosing and treating new health concerns. 

  • Referrals to specialists. 

  • Medication management. 

  • Coordination of all aspects of your care. 

Already, we’ve seen great success with our AdvantAge Health Centers, which are now open in Orange County and the High Desert at the following locations:

At Providence’s first senior-dedicated facilities, seniors enjoy faster access to appointments and extended visit times with their physicians, so medical staff can provide a “whole person” health care experience that considers all aspects of a patient’s life. 

Specifically, AdvantAge Health Centers provide each patient with: 

  • Personalized, compassionate care from a team trained to support seniors’ health and social needs. 

  • A skilled nurse care manager. 

  • An on-site health advocate to help with coordinating specialty care, hospital stays, insurance and general questions. 

  • Complimentary appointment transportation (at select sites). 

In addition, the AdvantAge team is fully integrated with the entire Providence family of resources for seamless, connected care, including our hospitals and centers of excellence, our extensive network of specialists in every aspect of health and lab, and our X-ray and other imaging services. 

The heartwarming feedback we’ve received on our new AdvantAge Health Clinics has been overwhelmingly positive, particularly when it comes to extended doctor’s visits. One cancer patient who struggled with getting medical supplies connected with a new vendor and received a year’s worth of supplies from our AdvantAge team, relieving the patient’s anxiety and providing the kind of personalized support that was needed.
