Partnership HealthPlan of California awards Providence Queen of the Valley Medical Center for Making a Difference

NAPA, Calif. (June 24, 2025) Partnership HealthPlan of California presented the CalAIM Make a Difference Award to Providence Queen of the Valley Medical Center CARE Network program representatives on June 4 in Fairfield. The hospital was among 16 organizations celebrated for transforming healthcare delivery across Partnership Health Plan’s 24-county service region.  

The CalAIM initiative focuses on delivering holistic, coordinated care that extends beyond traditional medical settings to address social determinants of health, mental health needs, and community-based support services. 

Along with the award, which recognized Queen of the Valley’s innovative approach to creating integrated, community-focused healthcare solutions, the hospital earned a $10,000 grant to support Queen of the Valley’s CARE Network, a program that supports and assists low-income residents of Napa County who have complex medical and social needs and are underinsured or uninsured. 

“This recognition reflects our commitment to our Mission of serving all, especially those who are vulnerable,” said Aura Silva, CARE Network Manager who received the award on behalf of Queen of the Valley earlier this month. "The CalAIM program supports treating the whole person, not just medical symptoms, and we're proud to be recognized for our work that furthers this movement.” 

The award acknowledges Providence Queen of the Valley’s success in building capacity to ensure  access to health services, quality care, and timely program implementation.
