Providence St. Joseph Medical Center is offering high school sports physicals on select dates in June, July and August. The cost of the physical is $30 (no insurance or credit cards accepted); all proceeds are donated to the Polson Booster Club to support athletic programs.
Prior to your child's exam, please visit the Montana High School Association website to download and print a Pre-participation Physical Exam Form.
2018 sports physical exam dates & times
- June 12 | 3:30-6 p.m.
- June 28 | 3:30-6 p.m.
- July 17 | 3:30-6 p.m.
- August 7 | 3:30-6 p.m.
- August 9 | 3:30-6 p.m.
