How we care for the poor and vulnerable
We ensure our community is healthy by providing programs that align with our mission of
caring for the poor and vulnerable:
Providence Community Health Investment Programs
Community Health Insurance Program (CHIP): Community Health Workers (CHWs) assisted
with 2,300 applications for state-suppor ted programs (Medi-Cal/Covered CA /CalFresh)
Creating Opportunities for Physical Activity (COPA): Provided physical activity programming
for 5,300 students across 10 schools in Lawndale, Wilmington, and San Pedro.
Emergency Department Community Health Workers: CHWs referred 3,556 emergency
department uninsured/underinsured patients to primar y care providers. Also referred 1,188
patients for assistance with Medi-Cal.
Emergency Department Homeless Navigators: CHWs assisted 643 homeless patients with
navigation and links to resources, 282 of which connected to a form of housing after discharge.
Health Education Classes: Five different series of classes ranging in topics, such as nutrition,
diabetes self-management, and mental health prevention. 428 people par ticipated in these
free programs.
