Partners for Healthy Kids Mobile Van: A school-based vaccination program for children that pivoted to COVID-19 vaccinations in response to the pandemic. 8,857 doses were given in 2021. Providence-Charles Drew University Community Health Worker Academy: Providence par tnered with Charles Drew University to create a new academy that trains CHWs to work in health care organizations. 27 CHWs completed 6-month paid internships at nine different hospital/clinic sites across Los Angeles Sexual Assault Response Team (SART): Both of our Torrance and San Pedro ministries operate SART centers. These centers provide compassionate, comprehensive, and culturally sensitive treatment for sexual assault victims through collaboration between the hospital, law enforcement agencies, forensic nurse specialists, and sexual assault counseling advocates. Vasek Polak Health Clinic: This outpatient clinic, located in Hawthorne, provides access to primar y care and walk-in clinic for minor illnesses with 3,100 visits this past year. An alternative to the emergency room for uninsured and underinsured patrons, the clinic provides for impor tant follow-up care after Emergency Depar tment visits, and offers additional free suppor t ser vices: Welcome Baby: Home visitation program by RN and parent coach for new mothers with 1,626 patients ser ved in 2021. • CHWs provided COVID-19 prevention/vaccination information, and personal protective equipment to 127,482 outreach contacts in Wilmington, San Pedro, Lawndale, and Hawthorne. • Our Mission extends beyond the walls of our hospitals and into the communities we ser ve. To help provide care in communities that have been dispropor tionately impacted by COVID-19, Providence works with valued community par tners, like The Gailen and Cathy Reevers Center for Community Empowerment and Lincoln Memorial United Church of Christ, to provide life- saving vaccines. Par tnerships like these are vital in making progress toward health equity across Southern California. Grants provided to local community partners Providence Little Company of Mar y awarded 15 grants, exceeding $3 million, to local non-profit community par tner organizations to help address areas impacted by access to care, homelessness, and mental health. Here's how some of these funds were used: • Access to Care » Increased operational suppor t to local safety net clinics: Harbor Community Health Center, Wilmington Community Clinic, South Bay Family Health Care, and Behavioral Health Ser vices. » Provided specialty care access for patients at Vasek Polak Health Clinic through Providence Medical Institutes and/or Medical Associates. » Initiated a NEW Providence Medical Institutes primar y care clinic adjacent to the San Pedro ministr y using the same model as the Vasek Polak Health Clinic in Hawthorne. • Homelessness » Added more recuperative (long term rehabilitation) care beds » Formed a homeless street outreach team with the Venice Family Clinic and South Bay Family Health Care. Responding to COVID-19 • Par tners for Healthy Kids pivoted to COVID-19 vaccinations in high-risk neighborhoods. 8,857 doses of vaccines were given. • Providence Little Company of Mar y Wellness and Activity Center ser ved as a COVID-19 testing site for the Wilmington community. • Set up pop-up vaccine clinics in Wilmington and Reseda to help bring the vaccine to areas where people did not have access to either the "my turn" website nor access to go to vaccine locations. 29 BE Our COMMuNitiEs' HEALtH PArtNEr
