INTRODUCTION ENVIRONMENTAL SOCIAL GOVERNANCE ABOUT US MEDICAID ENROLLMENT AND RENEWAL Losing or lacking health insurance has a serious impact on health and access to care. In 2023, Providence developed a multipronged approach to helping people in these situations. Our approach included: Proactive systemwide planning and coordination: We created a multidisciplinary steering committee to support planning and communication efforts for Medicaid renewal. Partnership with Providence Financial Counseling: We implemented a new digital tool to support financial counseling outreach, appointment scheduling and follow-up after emergency department visits for people without insurance. Multi-lingual patient communications: We deployed newsletters, social media, websites, fliers, emails and other multi-lingual communications to encourage Medicaid enrollment across our seven-state footprint. • 920,000 community members were reached through social media posts. • 1.73 million patients were reached through direct email and newsletters. • Over 53,000 pieces of collateral were distributed across our clinics. Resources for front-line caregivers: We created training programs and videos to prepare our caregivers to support patients through the renewal process; scripts and guides to help them answer common questions; and screen pop-ups to prompt hospital and clinic registration teams to remind patients to renew their coverage and to refer them for financial counseling if needed. Empowered community health workers: We developed a referral pathway with warm handoffs between community health workers and financial counselors to ease the process of applying for coverage. CEI metrics aligned with nationally set metrics (NQF, ADA, HEDIS). Data source is from Providence's care management tool, extracted on 1/23/2024 . Rolling 12 months ending December 2023, baseline is December 2022 for all measures except Colon Cancer Screen and Depression Screening and FollowUp CLINICAL EQUITY PERFORMANCE In 2023, Providence adult ambulatory care teams systemwide improved quality and reduced inequity for nearly all of the Indigenous people and people of color that we serve. We made measurable differences in reducing inequity with screenings for breast and colon cancers, depression and diabetes follow-up.
