Please note: For your safety, and to ensure that we have enough vaccine for those who made an appointment, we are not accepting walk-ins. We will only vaccinate age 65+ who have an appointment.

As a Providence patient age 65+, you have three options available to you to receive your COVID-19 vaccine:

Option 1

Hospital vaccine clinics: We have opened Providence patient vaccine clinics for those age 65+ at each of our hospitals* in Southern California and are currently accepting appointments. If you are a Providence patient age 65+, click here to view instructions on how to make an appointment. When you're finished reading the instructions, return to this page and click here to make an appointment. Please do not call the hospital or your doctor. Appointments can only be made online. If the appointment schedule is full, check back often. We add appointments daily.

Please note that you must bring proof of age at the time of your appointment. If you fail to bring a document that verifies you are age 65+ at the time of your appointment, you will be asked to reschedule.

Please bring one of the following with you to your appointment as proof of age:

  • Driver’s license
  • California ID card/Real ID from DMV
  • Passport
  • Birth certificate
  • Medical document/record from a medical provider that includes your name and age or date of birth.
  • Any official document that includes your name and date of birth.

For more information, please click here for Frequently Asked Questions.

Option 2

Your physician medical group (for those age 65+ with a serious medical condition only): If you are a medical group patient age 65+ and have a serious medical condition (e.g., cancer, congestive heart failure, etc.) you may receive an email from your medical group physician asking you to make an appointment to receive the vaccine. Only those 65+ with serious medical conditions will be contacted. If you are age 65+ and do not have a serious medical condition, see options 1 and 3 to receive your vaccine.

Option 3

Vaccine super sites: We encourage all individuals age 65+ to receive the COVID-19 vaccine where it is most convenient and readily available to you, such as vaccine super sites (e.g., Dodger Stadium, Orange County Fair Grounds, Disneyland, etc.). To learn about community vaccination super sites in your county for those age 65+, click on your county below.
