Community Health team provides CalFresh application assistance

The Los Angeles Region Community Health team provides CalFresh application assistance to eligible individuals throughout the South Bay and San Fernando Valley service areas. CalFresh is a countywide program that provides monthly food benefits to assist low-income households in purchasing the food they need to maintain a healthy lifestyle. If a patient has Medi-Cal, and is a U.S. citizen, a permanent resident, refugee/asylee, or a U or T visa holder, then he/she may qualify for CalFresh. Our Community Health Insurance Program (CHIP) specialists can help determine eligibility and navigate the application process.

CalFresh provides support to eligible individuals in several ways, including:

  • Extra money to buy groceries at local grocery stores
  • More access to buy fresh fruits and vegetables
  • CalFresh dollars and $10 worth of free food items from local farmers markets

The following information is needed for the application process:

  • Driver’s license/CA ID/Residency card
  • Social security cards for all of the household members
  • Birth certificate
  • Household income
    • Paycheck stubs and social security benefits (SSI/SSP recipients do not qualify)
    • Unemployment compensation award letter/stub
  • Bank statements
  • Utility Bills (electric, gas, water, trash and sewage, and/or telephone)
  • Rent receipts or mortgage bills
