Value-Based Care is an investment in healthcare payment models that reward providers and health systems for delivering high-
quality, cost-effective care. Rather than focusing on volume alone, this approach prioritizes measurable outcomes, appropriate
utilization, and coordinated care — ensuring patients receive what they need, when they need it, without unnecessary expense
or duplication.
Covenant Health Partners and Covenant Accountable Care Organization (ACO) lead Covenant Health's participation in Value-
Based Care models. Representing a coordinated network of providers and facilities, these entities manage care for defined
patient populations — groups whose outcomes and costs are measured to ensure accountability, quality improvement, and
financial stewardship.
Operating as independent nonprofit entities within the larger Covenant Health System, Covenant Health Partners and the
Covenant ACO embody our commitment to responsible stewardship. Since 2018, they have generated more than $29 million in
shared savings — resources that strengthen sustainability and support reinvestment in patient care and community health
initiatives.
The network includes 161 primary care providers and 411 specialist providers, collaboratively serving approximately 55,000
individuals. Through interdisciplinary coordination, data-driven performance measurement, and a shared commitment to
excellence, patients receive more connected, intentional care.
By aligning clinical excellence with fiscal responsibility, Covenant Health's Value-Based Care strategy advances quality
outcomes, protects long-term sustainability, and reflects our calling to serve with compassion, integrity, and purpose.
