Health Matters: Providence Holy Cross Medical Center | 7 W hen a family member has been through a long illness or is recovering from a traffic accident, it can be difficult for everyone involved. The Sub-Acute Care Center at Providence Holy Cross Medical Center specializes in caring for residents over 21 years old with complex medical needs. Whatever their situation, we know it takes a customized, compassionate approach to help them return to their highest level of independence and function. We spoke with Angie Lim Torres, DNP, director of Post- Acute Services, about how the center helps vulnerable patients—and their families—heal. What medical assistance does the Sub-Acute Care Center offer? We are committed to easing our patients' way and providing the best care possible for your loved ones. The unit offers 24-hour support in a homey atmosphere. This can include customized care, such as tracheostomy care and care for residents who need ventilator support, as well as inhalation therapy treatments, tube feeding, IV therapy, wound management and medication management. What kinds of caregivers are involved? Our interdisciplinary team includes the medical director, director of post-acute care, nurse manager of post-acute care, registered nurses, licensed vocational nurses, certified nurse assistants, restorative nurse aides, activity coordinators, physical, occupational and speech therapists, registered dietitians, pharmacists, social workers and a chaplain, among others. How do they minister to the whole person? We offer physical, occupational and speech therapy, as well as music, art and recreational activities. And we provide psychological and spiritual support tailored to the individual patient's needs. These transformative services help our patients heal—and help comfort their family members. Finally, what is the atmosphere like at the Sub-Acute Care Center? We really strive to make this a home for our residents. We call them residents, not patients. They have pictures in their rooms, can wear their favorite items of clothing and can bring their special blanket. We are so grateful to provide this specialized level of care to our residents and to be a part of their recovery.
