To reduce the risk of falling, bring this checklist to your pre op appointment so your care team can help anticipate your needs at home prior to discharge. Feel free to also take photos or a short video of your home to share with a member of your care team at your pre op appointment so your care team can help anticipate your needs at home prior to discharge TIMELINE AND TIPS - 4 TO 6 WEEKS BEF ORE YOUR SURGE RY CHE CKL IST: 1. Contact Insurance Make sure you understand your coverage. Your surgeon should have confirmed insurance authorization; call your surgeon's office to confirm this has been completed. Most insurance companies require pre-approval for an extended rehabilitation/skilled nursing facility stay. You may be required to pay some out of pocket expense for this care. We do not want you to incur any expenses that you may not have been planning for as a result of your stay. Contact your representative for authorization before surgery if you anticipate needing this coverage. 2. Pre-Register at the Hospital Expect a pre-registration phone call from the hospital. 3. Diet and Exercise Make food choices for healthy bones ( see appendix) Eat smaller portions avoid fried and greasy foods. Practice your exercises enclosed in this booklet ( see appendix) 4. Set-up Advanced Directives (Please see attached booklet) If completed, bring a copy of your advance directive to the hospital. If you do not have an advance directive, please discuss this with your Primary care Provider. Let us know if you need further instruction regarding advance directives; it is encouraged that all patients having surgery have a copy of their advanced directive in their electronic medical record.
